Her link matters beyond itself, since reaching rank 4 opens Hisano Kuroda's Death link, and her rank 10 item, the Hospital ID, lets you fuse Beelzebub. Persona 4 Revival keeps the Social Links, all newly voiced; Atlus has not said anything about Sayoko's.

How to unlock Sayoko: the hospital night job

From May 25 the hospital janitor job is open to anyone with Diligence 3 (raise it early: see Social Stats). Take the bus from Shopping District South on a Wednesday, Thursday or Friday evening and work the shift; on the second shift Sayoko starts the Devil link. Because it is an evening activity, it does not compete with daytime links, and Siliconera notes the hospital is open even on rainy days, which makes it a good use of a wet night.

When is Sayoko available?

Wednesday, Thursday and Friday evenings, per RPG Site and TheGamer, in any weather. Each shift pays 4,000 to 8,000 yen and raises Courage.

Points include the bonus for carrying a Devil Persona; RPG Site suggests drawing one in the Velvet Room before each shift. Prompts not listed accept any answer.

Sayoko flirts in the early ranks, but the guides describe no romance: RPG Site's guide offers no lovers choice at any rank.

Rewards

Rank 10 gives the Hospital ID, which lets you fuse Beelzebub in the Velvet Room, and every rank adds a bonus when fusing Devil Personas (see Persona Fusion and Best Personas). The link gives no battle skills. Its most practical reward comes early: rank 4 opens the Hisano Social Link, whose limited Sunday schedule makes an early start worthwhile.

Sayoko's link in Persona 4 Revival

Persona 4 Revival, out Feb 18, 2027, voices every Social Link event anew, per Atlus's Japanese site, and director Kazuhisa Wada told PCGamesN on Aug 26, 2026 the links are largely unchanged, with "little bits and pieces" added to some. Atlus has not said whether the hospital job or Sayoko's scenes change, and has not named her English voice. Every guide here is for Persona 4 Golden; none says how the PS2 Persona 4's version differs.
